Form to Change from the Default Contribution Rate on Joining - Click here to view the form to change from the default contribution rate on joining the GPP, which must be completed within two months of commencing employment. After this time you will only be able to change your ordinary contribution rate once a year, in April.

Alstom GLAS Expression of Wish Form - The Alstom GLAS Expression of Wish form lets us know who you would like to receive the benefits that will be payable if you die in service as a member of the Alstom GLAS. Click here to view a copy of this form. This form only applies to the Alstom GLAS - you need to complete a different form for the ALSTOM Ltd GPP (see below).

ALSTOM Ltd GPP Expression of Wishes Form - The ALSTOM Ltd GPP Expression of Wishes form lets us know who you would like to receive the benefits that will be payable from the ALSTOM Ltd GPP in the event of your death. Click here to view a copy of this form on Aviva's ALSTOM Ltd GPP website. This form only applies to the ALSTOM Ltd GPP - you need to complete a different form for the Alstom GLAS (see above).

Additional Voluntary Contributions (AVC) Form - If you wish to make an AVC instruction (to commence paying AVCs to the ALSTOM Ltd GPP, to change the level of AVCs that you pay or to cancel your AVC payments) at anytime except Annual Renewal at 6 April, you must complete the AVC Form.
