Getting the right balance

 

Need a form?
Here are all the forms you need to make the most of being a member.

Click on the links below to find the forms for the section you are interested in. Just print out the form you need by clicking on the relevant icon. Then fill it in and return it as instructed.

Increase your benefits?
To start paying contributions to the Additional Voluntary Contribution (AVC) Scheme, or to amend your AVC contributions or investments please complete the AVC Form (APS14) and submit it by following the instructions on the form.

Sort out payment of your death benefits?
Please fill in an Expression of Wish form (APS2) . The trustees take into account the information on your 'expression of wish' form when they decide who receives any cash sum that is payable when you die. You should always fill in a new form if your personal circumstances change (for example if you get married).

Leave the Alstom Pension Scheme?
If you are sure that you want to leave the Alstom Pension Scheme (other than because you are moving jobs), you need the Opting-Out form (APS5) . Use this form ONLY if you wish to opt out of the Alstom Pension Scheme and stay in employment with Alstom.

If you are leaving employment or retiring, your HR department will let JLT Employee Benefits know. But if you want to apply for ill-health retirement, please fill in Applying for Ill-Health Early Retirement form (APS12) , and send it to Alstom UK Pensions.

Keep us up to date?
If your personal details change (you move, or change name or marital status, for example), please let JLT Employee Benefits know. Use Changes in Personal Information form (APS10).

You might also need to update the information you have given in your Expression of Wish form (APS2) .

If you are going on maternity leave or starting to work overseas, your Human Resources Department will let JLT Employee Benefits know.

If you want an estimate of your benefits, let us know by filling in a Retirement Estimate Request form (APS8) . Remember, we send out annual benefit statements which may provide the details you need.

When you reach retirement, we will outline your options and ask you to decide. We will send you all the forms you need.
